Assessments
A health and social care assessment with one of our Adult's Teams is often the first step towards getting the help and support you need.
We have to collect information about you and find out what your needs are. This is called an assessment. The law says that we have to assess anyone who may need a service from us. Assessments vary from person to person. We will need to visit you to do the assessment. You will be closely involved in this and we will ask you what you think your needs are.
We aim to help vulnerable people who live in the borough to keep their independence, stay safe and enjoy a good quality of life.
How do I get it?
All Sandwell residents have the right to ask adult social care services to assess your needs for support and care.Contact your local Adults Team. We will ask some questions initially about your situation and where you live. Everyone is entitled to an assessment of their needs although you may not be eligible for services.
Your carer also has the right to their own assessment. A carer is someone who regularly provides care and support for a relative or friend.

When you get in touch with us, we first make sure that you are someone we are allowed to help.
We can arrange services for people with:
- learning disabilities
- mental health needs
- misuse substances
- physical disabilities
- sensory impairments (sight or hearing loss)
If appropriate, local health and social services teams will put together a package of support for you. This will be discussed with you and a written care plan produced. This may include services from both private and voluntary organisations.
If you need other services such as housing or benefits advice, you will be put in touch with the relevant local services.
You can request a new assessment and review of your care plan if your circumstances change.
Is there a charge?
You will not be charged for an assessment of your needs. All your health needs provided by doctors, nurses or hospitals are free but you may have to pay for part or all of the services provided by dentists, chemists or opticians. However, if as a result of the assessment certain services are offered, you may be assessed to pay for these.
