We provide services to support you, so that you can remain independent in your own home for as long as possible.

What services can we provide to support you at home?

Some of the services that you may find helpful include:
  • Day opportunities (day care) – getting out and about, meeting other people and taking part in social activities.
  • Telecare – electronic devices that will help you to live more safely in your own home.
  • Home support services – personal care and support at home.
  • Reablement services – short-term help while you recover from a trauma, such as a stroke or bereavement and are finding it hard to manage at home.
  • Respite care – a short break for you or your carer.
  • Community meals – a hot or frozen meal provided direct to you at home.

We also offer support to people leaving hospital to help them to come home as soon as possible.

If you feel you can no longer cope at home then you could move to supportive accommodation. This could include extra care housing, sheltered housing or residential care. However we aim to provide services to support you in your existing home as long as it is safe for you to remain there.
