Rehabilitation
Now known as 'Reablement'. The Reablement Service provides intensive, short-term help for people who have suffered some form of trauma, such as a stroke or bereavement. It will enable them to learn or relearn the skills necessary to continue living in their own home. Home support services may also be available for a short time during reablement.
This is usually done over approximately six weeks and may be carried out at the person's home, while in temporary residential care or at a day centre.
How do I get it?
Reablement services are usually only available after an assessment. You can contact one of our Adults Teams for advice.
Who does it?
There are a number of reablement services provided by a range of health and social care service providers. Contact our Adults Teams for details of what is available.
Is there a charge?
There are charges for most of our community care services. How much you pay will depend on your financial circumstances and the amount of help you need. We will tell you what the charges will be.
